In order to provide you with the best care possible, at the onset of your first appointment we ask that you complete a New Patient Registration Form, which will include questions about your health and an up to date medication list. We will ask you to update this information during each visit.
Your dental appointment is considered confirmed at the time of booking. This means we will be reserving time for you with Dr. Pham, and preparing your exam space by sterilizing all the equipment and materials required for your visit. Therefore, we request 48 hours notice if you are unable to attend your scheduled appointment. As a courtesy, we’ll phone you the day before your appointment to remind you of your booking time.
